Since the launch of the AB-HWCs, India has made significant progress in transforming several SHCs and PHCs.  The responsibility for this transformation lies with both central and state governments. This compendium documents the baselines for each state, the innovative models developed and also highlights the diversity of each state. It shines a light on the challenges and opportunities for each state in the implementation of the National Health Policy.  State level implementers can draw on each other's experiences to further strengthen healthcare delivery in the country.
